The Forms module is currently only available to invited schools.
Forms is a tool created to facilitate the creation and management of bespoke forms for purposes such as, but not limited to:
surveys
informationcollection
wellbeing plans
financial request approvals
questionnaires
anything else you can think of
The uses of Forms is effectively limitless given the customisation and functionality available.
